If full casket burial is your chosen service, we will meet with you to obtain the required information and discuss final preparations.  At the time of passing, we transport the deceased from place of death, whether it be home, hospital or nursing care facility.  Life Transitions staff will proceed with making  all of the necessary arrangements to move forward with burial.   We will liaison with your cemetery of choice ensuring that everything is in place.  In accordance with your family’s arrangements Life Transitions staff will facilitate and can assist with visitation, funeral and graveside services.

Staff & Administration Services

Life Transitions will meet with your family to discuss and finalize arrangements, arrange for cemetery assistance, assist in writing newspaper announcement (newspaper costs vary), confirm and arrange details concerning visitations, funerals, celebrations of life, and memorial and graveside services that accompany burial.

Documentation

Life Transitions will prepare documents for signatures required to comply with the Vital Statistics Act.   We will collect Medical Certificates from health professionals, submit necessary registrations and Permits and provide unlimited certified original Proof of Death Certificates.

Sheltering Remains

Life Transitions will hold your loved one in our care and perform the necessary preparations for burial.

Transportation Services

Transportation costs include staff and transportation for the deceased from place of death to our facility and also from our facility to the service location and/or cemetery.  For transfers 50 kms outside of Chatham-Kent/Lambton, there is an additional charge of $1.75/km (return).

Disbursements

Municipal and Provincial Death Registration fee.  Cemetery and burial fees.  Any outside service fees, eg. Clergy, Newspaper Notice, Flowers, etc.
